The initial evaluation will last 60-75 minutes and will take place in your home or via zoom if that is your preference.

Prior to the visit, you will complete an evaluation accelerator form that includes information about your medical history and goals for treatment.

The evaluation includes:

• An injury and health history review

• A movement/mobility assessment

• A review of findings

• A custom treatment plan and recommendations for future care

Follow up visits will last 60 minutes and will take place in your home or via zoom.

During this session we will discuss progress since the last visit and make modifications or progressions to your plan of care

We will begin with specific treatment to help you meet your goals and will update your home program.
